Choice medical billing service offers three fee schedules depending on specific level of services rendered for your practice. Percentage based on monthly billing revenues collected. Flat monthly rates are available upon request. Price based on volume, size and services.

*One time setup fee applies* (Due upon signing of contract)

• Client listing in database
• Setup new office practice for client & patients in practice management software database
• Enter insurance carrier data, fee schedules, ICD, CPT
• Clearinghouse setup enrollment, registration, contracts
• Any applicable enrollment forms required for carriers

*Fee schedule 1 claim submission only:

• Electronic and paper claim submission (1st, 2nd, 3rd)
• Correction and/or resubmission
• Claim submission summary report
• Notification of rejected claims within 24 to 48 hrs.
• Dedicated follow-up within 7 days
• Patient eligibility verification

*Fee schedule 2 includes the following:

• Electronic and paper claim submission (1st, 2nd, 3rd)
• Correction and/or resubmission
• Claim submission summary report
• Notification of rejected claims within 24 to 48 hrs.
• Dedicated follow-up within 7 days
• Patient eligibility verification
• Denial Management

• Includes fee schedule # 2 plus
• Tracking/claims management
• Payment posting/adjustments
• Weekly/monthly reports specified
• Patient statements (soft collections, up to 3 past due notices)
• Managing collections
• Patient inquires
